Kupuna Care vs. Med-QUEST in Hawaii: Which Program Covers Your Parent's Care

Hawaii runs two distinct systems for senior care support, and they serve different populations with different rules. Kupuna Care is a state-funded program with no income or asset limits. Med-QUEST is Hawaii's Medicaid managed care program with strict financial eligibility requirements. Understanding which one your parent qualifies for — and which one covers what they actually need — determines your entire care strategy.

The Core Difference

Kupuna Care keeps seniors in their homes with community-based services. It's designed for people who need help but aren't at nursing home level of care.

Med-QUEST (QUEST Integration LTSS) covers the full spectrum of long-term care — from home-based services to nursing home placement. It's designed for people who need a nursing facility level of care but may be able to remain in the community with enough support.

The financial divide is stark. A parent with $50,000 in countable savings may qualify for Kupuna Care but be over Med-QUEST's standard countable-asset limit. Do not assume the parent must spend down to $2,000; consult an elder-law attorney before transferring assets or choosing a spend-down strategy.

What Each Program Covers

Kupuna Care Services

Kupuna Care is administered through county Area Agencies on Aging and provides:

  • Adult day care
  • Home-delivered meals
  • Personal care (bathing, dressing, grooming)
  • Homemaker and chore services
  • Respite care for family caregivers
  • Transportation to medical appointments

Services are allocated based on an individual assessment, and the program uses cost-sharing — your parent may pay a portion based on ability. There's no set dollar maximum published statewide, but services are practical community supports, not comprehensive care coverage.

The Kupuna Caregivers Program is a separate companion program that pays up to $210 per week to contracted service providers (not directly to the caregiver) when the primary caregiver works 30+ hours per week outside the home.

Med-QUEST (QUEST Integration) Services

QUEST Integration covers far more, but only after the clinical and financial gates are cleared:

  • 100% of nursing home care (semi-private room)
  • Home and Community-Based Services (HCBS) — personal care, home health, adult day health
  • Community Care Foster Family Home (CCFFH) placement
  • Expanded ARCH (E-ARCH) placement
  • Medical equipment, supplies, and transportation
  • Case management through a QUEST Integration managed care plan

For married couples where only one spouse applies, spousal impoverishment protections allow the community spouse to retain up to $162,660 in countable assets and receive income transfers up to $4,066.50/month.

When to Use Each Program

Start with Kupuna Care if:

  • Your parent needs help at home but doesn't require nursing-level care
  • Your parent has savings or income above Med-QUEST limits
  • You need adult day care, meal delivery, or respite while maintaining the parent at home
  • You're buying time while the parent's condition evolves

Apply for Med-QUEST if:

  • Your parent meets (or is approaching) the DHS 1147 clinical threshold
  • Assets are at or near the $2,000 limit
  • You're looking at nursing home placement, CCFFH, or high-intensity HCBS
  • The parent's care costs would otherwise exhaust savings within months

Use both strategically: Kupuna Care can bridge the gap while you're planning a Med-QUEST application. A parent receiving Kupuna Care adult day services can continue those services while you work with an elder law attorney on Medicaid planning — positioning assets within the look-back rules, establishing a family caregiver agreement, or protecting the family home.

Common Mistakes

Assuming Med-QUEST covers everything immediately. The application and eligibility-review process takes time. The DHS 1147 clinical assessment must be completed by a licensed provider and reviewed by HSAG. During the review, confirm how interim care will be paid rather than assuming Med-QUEST coverage has started.

Ignoring the look-back. An uncompensated transfer for less than fair market value within 60 months may trigger a penalty period. Hawaii's 2026 penalty divisor is $8,850.00 — so an uncompensated $44,250 transfer produces a 5-month period where Med-QUEST won't pay for care.

Not applying for Kupuna Care because it seems small. Adult day care alone costs $2,383–$2,551/month in the product research range and may be partly offset by Kupuna Care, subject to assessment and any applicable cost-sharing. There's no asset limit or estate recovery for Kupuna Care.

Waiting too long for either. Program availability and waitlists vary by county, especially on neighbor islands. Apply as soon as your parent meets the clinical criteria, not when you're in crisis.
